In-Home Care Planning Checklist
Manage care as a portfolio, not a service
No family can buy all the care, all the time. Full-time non-medical home care now runs about $80,000 a year at the national median. The families who do best put paid hours where they matter most, and let family, community and technology cover the rest under one shared plan.

Work through the nine areas in order. In a crisis, start with areas 1 to 3 and come back for the rest. Revisit the checklist every 3 to 6 months, and after any hospital stay, fall, new diagnosis or caregiver change.

Why home care fails: the 9 biggest gaps
- No one owns the whole plan. Families end up as the system navigators, juggling doctors, agencies, medications and appointments.
- Cost does not match need. Medicare does not pay for custodial care. Most families must ration paid hours.
- Caregiver shortages and turnover. Every state reports home care shortages; fewer than half of direct care workers stay a year.
- Broken continuity and poor fit. Consistent caregivers are linked to fewer falls and better function, yet continuity is weakest for those who need the most care.
- Family caregiver overload. Nearly 1 in 4 family caregivers give 40+ hours a week; 1 in 5 report fair or poor health.
- Untrained medical tasks. About half of family caregivers handle medications, wound care or injections, mostly self-taught.
- Missing legal readiness. Only about 1 in 3 older adults have a durable financial power of attorney.
- Safety and trust risks. Financial exploitation, weak screening and unmodified homes raise the risk of harm.
- The senior's voice gets lost. Care that only covers tasks misses routine, purpose, dignity and connection.
