Before You Start Read first

Important: keep private details out of this checklist

This checklist tracks whether something is done and what happens next. It is meant to be shared with family and brought to coaching sessions, so do not write sensitive information on it, including:

  • Social Security, bank, credit card, brokerage or insurance policy numbers
  • Passwords, PINs, security answers, alarm or gate codes, safe combinations
  • Medicare or medical record numbers, diagnoses or medication details

Write where the information is kept instead (for example, "In Life Record binder, tab 4" or "In password manager"). Keep the details themselves in the SilverBeacon Life Record or another secure place.

Check with a professional

SilverBeacon Aging Readiness Coaching helps you get organized and coordinate with your own advisors. It is not legal, tax, financial, insurance or medical advice. The descriptions in this checklist are general guidance, and laws, forms and benefit rules vary by state and change over time. Before you sign, change or rely on any document, account or care decision, confirm it with a qualified professional such as an attorney, CPA, financial advisor, insurance agent, physician, pharmacist or nurse. Items marked Needs must be completed or reviewed by one.

P1 / P2 boxes not started    / in progress    ✓ done    N/A does not apply. For a shared item (a joint account, the house), mark both boxes.
